Case Management

Helping older adults live independently in their own homes for as long as possible is central to Senior Friendship Centers' Social Services and Case Management program. Case manager skills are matched with service options available through funded programs at State and Federal levels.

When Do You Need an Eldercare/Case Manager?

If you are concerned about what would happen to you or a loved one in the event of an unexpected illness or accident or if you or your loved one is experiencing:

  • Difficulty with household activities

  • Problems with money management or legal issues

  • Poor eating habits

  • Unusual behavior such as disorientation, getting lost walking or driving

  • Loss of interest in social activities
  • Unsafe behavior, falling, unexplained bruises, appliances left on, etc.

  • Balance problems with refusal to seek medical attention or use assistive devices

  • Forgetfulness in taking medication

  • Increased consumption of alcohol and/or prescription medication

Peace of mind increases knowing that your loved ones are being looked after and cared for when you cannot be there.

Eldercare/Case Management fees are based on an hourly rate for visits and/or coordination of services. Services are billed at 15-minute intervals. The Initial phone conversation is provided free of charge.
