Helping older adults live independently in their own homes for as long as possible is central to Senior Friendship Centers' Social Services and Case Management program. Case manager skills are matched with service options available through funded programs at State and Federal levels.
When Do You Need an Eldercare/Case Manager?
If you are concerned about what would happen to you or a loved one in the event of an unexpected illness or accident; or, if you or your loved one is experiencing:
- Difficulty with household activities

- Problems with money management or legal issues

- Forgetfulness in taking medication

|
- Unsafe behavior, falling, unexplained bruises, appliances lift on, etc

- Balance problems with refusal to seek medical attention or use assistive devices

- Unusual behavior such as disorientation, getting lost walking or driving

- Poor eating habits
|
Our goal is to provide affordable services for older adults and their families by:
- Identifying your options

- Accessing services
|

|
|
| Through the use of volunteers and coordination of local community resources, we address the multiple needs of our elders to avoid their premature institutionalization. |
