Case Management

Helping older adults live independently in their own homes for as long as possible is central to Senior Friendship Centers' Social Services and Case Management program. Case manager skills are matched with service options available through funded programs at State and Federal levels.

When Do You Need an Eldercare/Case Manager?

If you are concerned about what would happen to you or a loved one in the event of an unexpected illness or accident; or, if you or your loved one is experiencing:

  • Difficulty with household activities

  • Problems with money management or legal issues

  • Forgetfulness in taking medication

  • Unusual behavior such as disorientation, getting lost walking or driving

  • Poor eating habits
  • Unsafe behavior, falling, unexplained bruises, appliances lift on, etc

  • Balance problems with refusal to seek medical attention or use assistive devices

  • Loss of interest in social activities

  • Increased consumption of alcohol and/or prescription medication

Eldercare/Case Management fees are based on an hourly rate for visits and/or cooperative services. Services are billed at 15-minute intervals. The initial phone consultation is free of charge.
